Scaling person-centred practice training to improve service delivery consistency.

Background: 

Numerous large organisations have embedded HCM into their health services. In 2012 Alberta Health Services (AHS) conducted a world-wide search to find a scalable training program that assisted clinicians to provide self-management support to clients.  HCM was chosen as it aligned with key AHS initiatives, policies and documents. They found that other existing educational offerings were not set up to address how providers could embed patient-centred behaviour change support into their practice. AHS continues to provide other training offerings alongside HCM and reports better uptake of concepts into practice.

HealthChange Associates’ Pathway to Independence

HealthChange Associates worked in partnership with Alberta Heath Services to:

  • Deliver two Train the Trainer programs so HCM courses can be offered in-house
  • Train and mentor two HealthChange® Master Trainers to drive implementation and organisational change
  • Re-design numerous services. Assisting with changes to intake processes, care planning, documentation and electronic medical records, and evaluation
  • Create a Licensing Agreement to enable autonomy

Impact: 

 The Osteoarthritis Chronic Care Program (OACCP) compared their results to the previous year after implementation of HCM: 

  • Improved ability of patients to self-management: Surgical deferrals due to poor self-management reduced from 51% to 34% 
  • Reduced cardiac-related deferrals due to poor self-management alone declined from 29% to 10% 
  • High levels of patient engagement: Telephonic follow up with patients showed high levels of engagement and continued effective self-management, many avoiding surgery altogether 

Impact:

The Canadian HealthChange Team have trained over 8,000 clinicians since 2013 and worked on numerous implementation projects. These have been across a variety of areas: Community Rehabilitation, Primary Case Networks and Family Care Clinics, Chronic Pain and Bariatric programs, as well as Healthy Living and Falls Prevention programs.

The HealthChange Team have collaborated with the University of Alberta, Faculty of Pharmacy and Pharmaceutical Sciences to embed HCM training into the teaching curriculum. 92% of providers agreed that HealthChange training enables more person-centred care.

The Wood Buffalo Primary Care Network Executive Director described the impact as: It is now “the way we do business”. From the moment someone walks in the door until they evaluate the program.  All areas are involved: intake, care planning, documentation, and evaluation.

One Canadian HealthChange® Trainer reported that “Now my patients will benefit from my expanded practice of being consistent and systematic, for more predictable outcomes.”
