Background: 

Murray Primary Health Network (Victoria) identified a need for Chronic Disease Management Health Coaching Training across the region in 2020. HealthChange Associates was selected by tender to deliver a comprehensive training package across the catchment area. The focus was to provide practical training to Nursing and Allied Health Professionals to manage clients with chronic and complex conditions and demonstrate reorientation of practice.

What was delivered?

  • 148 professionals trained over 6 months (face-to-face & online options)
  • Participants completed the Essential Toolkit Course and had the option of attending an Advanced Workplace Application 1-day workshop.
  • Reporting on relevance of content, facilitation, confidence to apply skills (before and after), practice changes made.

Impact: 

 The Osteoarthritis Chronic Care Program (OACCP) compared their results to the previous year after implementation of HCM: 

  • Improved ability of patients to self-management: Surgical deferrals due to poor self-management reduced from 51% to 34% 
  • Reduced cardiac-related deferrals due to poor self-management alone declined from 29% to 10% 
  • High levels of patient engagement: Telephonic follow up with patients showed high levels of engagement and continued effective self-management, many avoiding surgery altogether 

Impact:

  • 96% of participants would recommend the training
  • Clinicians increased their confidence to apply the skills by 21% after the advanced course
  • Practice changes included: altering their role explanation, use the HealthChange Practice Principles and Essential Techniques, altering health literacy delivery and documentation.
